The next phase of the Patient Safety Incident Management System (PSIMS) project will include potential development of a mortality review recording module (Alpha and Beta phases).

PSIMS will replace the National Reporting and Learning System (NRLS) and the Strategic Executive Information System (STEIS) with a successor that supports efforts to reduce harm through better data collection and analysis . PSIMS and the Patient Safety team in NHS England and Improvement are well-suited for hosting any mortality review recording tool, in line with principles of infrastructure/technology re-use and sharing of learning.

Why We Are Consulting

We would like to understand your user needs around mortality review recording, to inform development.

While many of you participated in engagement sessions around one year ago, this survey will capture any developments since that time, in a format that supports onward analysis.

We would also like to invite participation in ongoing user engagement activity (please provide your relevant details if interested).

What Happens Next

Your responses will help inform our understanding of mortality review recording goals, constraints and opportunities, before moving in to the Alpha phase where we will build prototypes and test different ideas.
